A case of parathyroid carcinoma, admitted at the Intensive Care Unit for coma, is described. Following the detection of severe hypercalcemia and of a cervico-mediastinal mass, surgical procedure had to be planned as an emergency: the operation was then performed within the first 24 hours. The post-operative evolution showed a dramatic improvement in general conditions and the calcium values dropped within the range of normality in six days. Personal experience and a literature review about the parathyroid carcinoma are discussed.
