The management of malignant pleural mesothelioma

The epidemiology, presentation and diagnosis of malignant mesothelioma is reviewed. Treatments that have had curative intent are discussed. The importance of review by an experienced multi-disciplinary team is stressed. Even when treatments are intended to prolong life, the need for active symptom control is paramount. Relief of breathlessness, by treating co-morbidity and dealing with pleural fluid collection is considered. Pain relief by medication and cordotomy is reviewed, and the prevention of tumour seeding into drain tracks by local radiotherapy is highlighted. The patients need counselling, support and medico-legal advice.
