New minimally invasive surgery technique, for the treatment of angular deformity of the lower limbs in children and adolescents

Aim. The aim of this study was to evaluate the effectiveness of a new surgical technique used in the treatment of the axial deviations of the inferior limbs in children, through the analysis of the pathological situation, of the effectiveness of the surgical technique and the outcomes.
Methods. This study included 59 children operated for angular deformity of the lower limbs, treated with this surgical technique between December 2006 and July 2010, at the Pedriatric Orthopedics Departement of Novara; mean age of the patients was 10.55 years (range 3-16). All the patients were treated with surgical treatment of tibial e/o femur, hemiepiphysiodesis (through the application of an “8” plate) inside, in the case of valgus, outside in the case of varus.
Results. In 17 patients the plates were removed, and in all the cases the anatomical axle of the inferior limbs was restored; the realignment was obtained within 16 months. Correction is in progress in those patients still in treatment (71%) and the evaluation of the evolution is guaranteed from a clinical-instrumental checking. For what concerns complications, in two cases (2.7%) the breakup of a screw at the device removal was detected, and in 10 patients a modest and transitory difficulty of the flex-extension of the knee in the immediate postoperative time was notice, solved with the aid of rehabilitation; there were no complications due to infection.
Conclusion. Our clinical experience shows that the hemiepiphysiodesis with eight-plate represents a simple and effective treatment for the correction of the angular defects of the inferior limbs in all those patients where the cartilages are still vital.
