In this paper we briefly review our extra-articular technique for reconstructing the anterior cruciate ligament, the outcome of over thirty years of applying the various techniques that have been adopted historically for reconstructing this ligament. We also explain the reasons and, briefly, the biomechanical premises that led us to use this approach, encouraged by a favourable review of the series which evidenced more than flattering results even if compared with the results obtained in intra-articular reconstructions. We refer the reader to another paper for an examination of an even greater sample of patients to further test the results of this technique which is easy to carry out and readily reproducible.
