Surgical treatment of subacromial impingement syndrome. Comparison of arthroscopic and open decompression results

Aim. Surgical treatment of subacromial impingement syndrome is based on 2 techniques of subacromial decompression (SAD): open and arthroscopic. The aims of this work have been: 1) to make a comparison of arthroscopic and open decompression and find differences in early, middle and long-term follow-up; 2) to analyse characteristics of both techniques according to the Constant scoring system.
Methods. A total of 80 patients divided into 2 groups according to the surgical technique (A: open SAD; B: arthroscopic SAD) have been studied. Sex, age, length of follow-up and score were assessed for each patients.
Results. Group A: 28 patients (18 women, 10 men), mean age 52.62 years. Seven cases of early follow-up, 15 cases of middle and 6 of long-term follow-up. Mean score has been 82.5 (a mean score from 80% to 90% with respect to reference value has been obtained for all Constant scoring system items). Group B: 52 patients (27 women, 25 men), mean age 49.22 years. Nine-teen cases of early follow-up, 26 cases of middle and 7 of long-term follow-up. Mean score has been 95.15 (a score equal or higher than 90% with respect to reference values has been reached for all Constant scoring system items).
Conclusion. Subacromial decompression is an effective way to treat subacromial impingement, mostly in cases of middle and long-term follow-up, when medical and physiotherapeutic treatments fail. Superiority of arthroscopic decompression, due to less invasiveness, is mostly evident in cases of early follow-up, but also in middle and long-term follow-up and for all Constant scoring system items.
