Phototherapeutic keratectomy for fish eye disease

We have described the clinical case of a patient with fish-eye syndrome treated with bilateral phototherapeutic keratectomy (PTK). Female, 48 years, affected by fish eye disease, characterized by hypolipoproteinemia and decreased activity of lecithin cholesterol acyltransferase, presented with blurred vision, photophobia and ocular discomfort. The typical corneal lipid deposits were localized in the extreme and middle corneal periphery, the keratoscopy was characterized by rings’ distortion. After an accurate clinical assessment the patient underwent to bilateral PTK. There was a visual acuity improvement immediately and during the entire follow-up. We did not have relapses or complications. PTK is a good change for treatment and visual rehabilitation in this kind of corneal dystrophy. It is a conservative and minimally invasive method, it can be repeated (compatibly with pachymetry) and does not exclude the possibility of a corneal transplant.
