DMLE and diet supplementation, clinical evidence from a retrospective automated analysis of OCT scans

AIM: Purpose of the study was to assess the automated analysis of spectral domain optical coherence tomography (SDOCT) scans as a diagnostic indicator for disease progression in patients with non-neovascular age-related macular degeneration (AMD), receiving antioxidants, macular pigments, omega 3 (DHA) and resveratrol supplementation.
METHODS: A retrospective study of OCT records was performed. We selected all AMD patients (AREDS 2 or 3 category) in at least one eye, who should have got regular 6-months follow-up visits with OCT scan in the last 12 to 36 months. All patients should have also received antioxidants, macular pigments, omega 3 (DHA) and resveratrol supplementation (Nutrof Total®). All the scans should have been performed and stored with the same equipment (Topcon® 3D OCT-1000). Automated analysis was performed with a TOPCON® software (Macula Drusen Analysis - 3D OCT -1000 MK2 -version 3.51), to compare total area, total volume and number of suspected drusen >125µm vs. baseline scan.
RESULTS: Seventeen patients (22 eyes) fulfilled the inclusion criteria for the longer follow-up period of 36 months, 23 patients (37 eyes) for a 24-month follow-up period and 31 patients (48 eyes) for a 12-month follow-up period. The drusen analysis detected no statistically significant change vs. baseline in number, total area and total volume of drusen over all the follow-up periods (paired samples T test). In the 22 eyes with the longer follow-up period (17 patients 76.1+8.5 years old; 14 F/3 M) the number of drusen >125 µm resulted unchanged vs. baseline after 36 months (mean 9.05 vs. 9.36; P=0.69); the mean total area and total volume of drusen resulted unchanged too vs. baseline (P=0.067 and P=0.101, respectively). OCT authomated analysis results were in line with the global clinical assessment, since AMD was stable or improved in 16 and 4 eyes respectively and worsened just in 2 eyes. Visual acuity too resulted stable all the eyes, in one case an improvement was observed. Similar results were observed in the subgroup of patients with shorter follow-up period (12 and 24 months).
CONCLUSION: Automated analysis of OCT scan provides a helpful quantitative assessment in AMD and improves clinical monitoring of AMD evolution. In our selected population receiving antioxidants, macular pigments, omega 3 (DHA) and resveratrol supplementation the disease resulted stable up to 36-months.
