Exercise induced left bundle branch block in a cyclist. A case report

The case report tells about a 46 years old cyclist, in apparently healthy state. During a cycloergometer stress test, played in order to reach agonistic suitability, an heart rate related left bundle branch block appeared. No cardiomiophathy was revealed by echocardiography, while the scintigraphic myocardial stress test showed a reversible captation defect in the inferior side. The coronarography revealed a myocardial bridging on IVA’s apical tract, able to determine a systolic stenosis of 60%. This case report shows the necessity of the monitorization of the stress test, and that a myocardial bridging may be a cause of rate depending left bundle branch block. The regular practise of non agonistic aerobic sport, under clinical and instrumental control, may be useful as preventive and rehabilitative instrument for this patient.
