Metabolic complications associated with low adherence to drug therapy are emerging after prolonged exposure to antiretroviral therapy, highlighting lipodystrophy. In addition to the impact on self-esteem, the redistribution of body fat is related to disorders of glucose metabolism and dyslipidemia. The purpose of this study was to evaluate anthropometric and biochemical variables changes related to lipodystrophy. The evaluation was made after physical exercise program and nutrition advising as a strategy for adherence to HIV positive treatment. This research study reports the case 48 years old male with a diagnosis of HIV infection since 2000 and under the use of anti-retroviral therapy. The intervention lasted 16 weeks and it was based on physical training and nutritional advice. The training program was characterized as a concurrent, consisting of endurance training, alternated with aerobic training in the same workout. The nutritional advising was based on a 24 hour dietary recall, considering the usual recall. Anthropometric measurements, biochemical examinations and photographic records of each individual were performed. In addition, it was observed patient behavior, attendance and reports to the training, focusing on the self-esteem. The physical exercises and the adoption of healthy eating habits contributed significantly in the redistribution of the body fat. Particularly reduction of abdominal fat and triglycerides, as well as in the relationship of psychological and social character of the participant due improvement of the body image.
