Stress fractures are fatigue fractures occurring in normal bone, often in athletes performing repetitive exercise. We report the case of a 20-year-old female runner who presented with insidious-onset lower back pain. On MRI scanning, a stress fracture through her left sacral ala was noted. She was managed conservatively and returned to full activity after 3 months. This rarely reported condition has been linked to the ‘female athlete triad’ of associated features, namely amenorrheoa, osteoporosis and an eating disorder, none of which our subject had.
