The aim of this report is to present a case of an aggressive keratocystic odontogenic tumor, affecting the left side of the mandible of a 33 year old man. The patient presented some particular features that challenged clinical and imagenological diagnosis such as size of the lesion, asymptomatic abscess and multilocular pattern. Incisional biopsy histopathological result was inconclusive. The patient was treated with hemimandibulectomy and a titanium plate with condylar prothesis was placed to restore facial contour and physiologic function. Excisional biopsy histopathological result confirmed keratocystic odontogenic tumor. Aggressive lesions such this one, demands a non-conservative approach, may have different outcomes related to the wide spectrum of therapeutic modalities and close follow-up is mandatory.
