Despite the rich vascularization and complex circulation, metastasis to the penis is rare. The most common symptoms are penile induration, swelling and palpable nodular lesions on penis. These patients have a poor prognosis receiving various treatment modalities. The appearance of disseminations is still controversial and there is not an efficacious therapy. Herein, we present a patient who admitted with the chief complaints of hematuria. After clinical, radiological and pathological evaluations, the patient was diagnosed as a very rare cause of hematuria; penile metastasis of prostate cancer. In literature, this is the first case of prostate cancer metastasis to penis which is presented with hematuria. In follow-up period of him, although he was treated with chemotherapy, he died 10 months after the diagnosis.
