Empty sella syndrome (ESS) is an asymptomatic disorder that is incidentally discovered during neuroradiologic imaging tests. The affected patients usually have normal pituitary function, although the anterior pituitary and rarely posterior pituitary gland may be involved. Here we reported a 41 year old woman with a history of long standing central diabetes insipidus (CDI) that is resulting from empty sella syndrome (ESS) and is associated with adrenal insufficiency. We recommend that in all patients with preoperative diagnosis of CDI, pituitary MRI should be done and if the diagnosis of ESS was made the complementary pituitary function tests should be performed.
