Primary sclerosing cholangitis and inflammatory bowel disease

AIM: Primary sclerosing cholangitis is an autoimmune liver disease, characterized by a cholestatic serum enzyme pattern. It is associated with inflammatory bowel disease. The aim of this study was to describe the clinic characteristics of our patients with primary sclerosing cholangitis and which of the two inflammatory bowel diseases is more frequently associated with cholangitis.
METHODS: We have analyzed patients’ clinic presentation, imaging studies, treatment, laboratory and endoscopic characteristics in the moment of their first admission in our Institute.
RESULTS: In the last 19 years 28 patients with primary sclerosing cholangitis were hospitalized for the first time in our Institute: 14 males, median age 52 years, range 17-85. All the patients had cholestatic serum enzyme pattern. The diagnosis was made with endoscopic retrograde cholangiopancreatografy (42%), liver biopsy (50%), or magnetic resonance cholangio-pancreatography (8%). The 75% of the patients submitted to colonoscopy was affected by ulcerative colitis, nobody by Crohn’s colitis. Three of the patients with ulcerative colitis had colon carcinoma also.
CONCLUSION: The primary sclerosing cholangitis is a rare autoimmune liver clolestatic disease with very high association with ulcerative colitis and colon carcinoma. So they need careful follow-up.
