The perianal Paget disease is a rare condition. It may be associated with an underlying carcinoma of the rectum, which may exist simultaneously or may appear later. Until 1993 only 114 cases were reported in the international bibliography. We report two cases caracterized by irregular erythematous perianal plaques. The lesions did not respond to topical antinflammatory treatment. So, skin biopsy was taken and histological examination showed typical findings of Paget’s disease. No carcinoma of the rectum was found in both the patients. A broad surgical excision of the affected skin was performed and the patients has been well until now and are in follow up every 6 months.
