The assessment of anticardiolipin antibodies in ectopic pregnancies

Aim. To investigate the association between anticardiolipin antibodies(aCL)and ectopic pregnancies.
Methods. The study group consisted of 86 women with ectopic and 40 women with normal intrauterine pregnancy admitted for legal elective abortion. Measurements of serum levels of IgG, IgM antibodies against cardiolipin (aCL), human chorionic gonadotropin and progesterone were made.Information regarding gestational age, maternal age, parity, surgical and medical treatments of ectopic pregnancies were collected.The two groups were compared for demografic characteristics and serum levels of autoantibodies and hormone levels. For estimation of sensitivity, specifity, and predictive values receiver operating characteristic(ROC) curve analyses were performed.
Results. The mean age and granty were higher in control group. 34 cases of ectopic pregnancies had initial surgical management and 4 of the 52 cases of ectopic pregnancies who received single dose methotrexate initially needed laparatomy due to tubal rupture. The comparison of aCL antibodies between the two groups showed a significantly increased IgM levels in cases (p=0,000), but IgG levels did not differed significantly(p=0,717). The mean levels of hormones were significantly increased in the control group (p<0,001).The area under the ROC curve with IgM is 0,785 and the cut-off level is 3,15U/ml, sensitivity 59,3%, specifity 92,5%, negative predictive value 51,4%, positive predictive value 94,4%.
Conclusion. There is an overlapping in antibody titers of intrauterine and ectopic pregnancies and the ROC curve analysis showes that the sensitivity is low with this cut off levels.
