We report a rare case of bilateral primary osteonecrosis of the femoral condyle and tibia plateau. The patient was a 53 year old women with a 18 months history of pain in her right knee. Radiographs and magnetic resonance imaging of the knees showed bilateral osteonecrosis of the femoral condyle and tibia plateau. Treatment has been conservative so far.
