Myofibroblastoma of the breast. Report of a case

Within the contest of stromal lesions of the breast the existence of myofibroblastoma must be considered, because less than 50 cases have been reported in literature. The case of a 50-year-old woman in whom a breast myofibroblastoma was diagnosed histologically and immunohistochemically is reported. The clinical breast mass measuring 3 cm was well-circumscribed mammographically and ultrasonographically, without suspicion. Extensive surgical excision was advised because of progressive volumetric increase of the mass for 1 year. Histological examination demonstrated a tumor consisting of spindle-shaped cells and broad bands of hyalinized collagen, but no epithelial components were observed. Immunohistochemically the tumor cells were diffusely positive for desmin and alpha smooth muscle actin and a diagnosis of myofibroblastoma was made. At 18 months follow-up, the patient was aesthetically well and free from recurrence. So that wide excision proves to be the right procedure. The diagnosis of myofibroblastoma is only by pathologist and the therapy must avoid performing unnecessarily extensive procedures.
