Diverticular disease of the right colon is frequent among Eastern populations, but very rare in other countries. The author reports four cases of acute diverticulitis of the right colon undergoing emergency surgery. In all cases surgery was undertaken in the presence of clinical symptoms of acute appendicitis, and the diagnosis, subsequently confirmed by histological analysis, was only made intraoperatively.
In those forms characterized by inflammatory pseudotumour, doubts may arise regarding the interpretation of the carcinoma. Depending on the objective characteristics and the spread of the inflammation process, the surgeon may opt for conservative (diverticulectomy) or radical (ileocecal resection, right hemicolectomy) treatment. Operative mortality and morbidity are usually null.
