Training program and walking with an ARGO in SCI. Clinical case report

BACKGROUND: The pur­pose of ­this ­study was to deter­mine the effi­ca­cy of a per­son­al­ised walk­ing train­ing pro­gram (WTP) on Thoracic para­ple­gics sub­jects walk­ing ­with an Advanced Reciprocating Gait Orthosis (ARGO).
METHODS: Three vol­un­teer ­male para­ple­gic sub­jects, T6-12 ­with com­plete spi­nal ­cord inju­ry, 24-31 ­year old, par­tic­i­pat­ed to an stan­dard reha­bil­i­ta­tion pro­gram (SRP) ­prior to the com­ple­tion of the ­full ­ARGO walk­ing train­ing pro­gram (­AWTP). The ­time ­elapse ­from the com­ple­tion of the SRP and the initi­a­tion of the ­AWTP was 11-31 ­months. The ­AWTP was fin­al­ised to ­improve: 1) mus­cu­lar ­strength and car­di­o­res­pir­a­to­ry con­di­tion; 2) bal­ance con­trol and pos­tu­ral reac­tions. Speed, ener­gy ­cost, ener­gy con­sump­tion and ­main car­di­o­res­pir­a­to­ry param­e­ters ­were reg­is­tered dur­ing ­ARGO walk­ing ­with crutch­es at a ­self cho­sen ­speed, ­before and ­after train­ing.
RESULTS: Walking ­test ­after the ­AWTP ­revealed a ­decrease in ener­gy ­cost and an ­increase in ­free walk­ing ­speed.
CONCLUSIONS: These ­results sug­gest ­that an inten­sive and spe­cif­ic walk­ing train­ing pro­gram on tho­rac­ic par­a­ple­gios sub­jects deam­bu­lat­ing ­with an ­ARGO is ­well tol­erat­ed and can pro­vide fur­ther improve­ment in the walk­ing effi­cien­cy com­pared to the stan­dard treat­ment.
