Pemphigus herpetiformis (PH) is a rare clinical variant of pemphigus which was described for the first time more than 50 years ago. Patients affected with PH usually present with clinical features resembling dermatitis herpetiformis, whilst the histopathological and immunopathologic features of the disease are more similar to pemphigus vulgaris. PH has been largely reported to be associated with pulmonary and esophageal cancer over the last thirty years. We present a case of PH in association with esophageal cancer. It has not been validated yet whether these combinations occur accidentally or whether they could represent variants of paraneoplastic pemphigus which have to be considered distinct from the classical five forms already characterized.
