Dermoscopy is a very effective technique for the diagnosis of skin tumors, melanocytic or not, but also for many non-neoplastic acquired cutaneous lesions. In this paper we describe dermoscopic features of a benign but rare cutaneous lesion such as acquired digital fibrokeratoma; the lesion was appeared three years before on the third finger of the left hand of a 63 year old man. Dermoscopic exam showed a central homogeneous rosy-white area, surrounded by thin dark linear concentric structures with several dotted vessels within. Surgical excision with histopathological exam was recommended because the presence of dotted vessels didn’t let us to exclude the diagnosis of a ipo/amelanotic melanoma with high Breslow’s thickness.
