The use of an anchor flap is proposed for the reconstruction of extensive loss of substance of circular form from the nasal tip consequent on the removal of a voluminous base-cell carcinoma; the name of the flap derives from its final shape which calls to mind a reversed anchor. The anchor flap is an alternative for the repair of defects in the roundness at the tip of the nose, permitting a valid reconstruction in just a one-stage operation; esthetic results are satisfactory because the scars are not too much in evidence on the join lines between the different esthetic units of the nose and due to the use of skin of colour, texture and thickness very similar to those of the original skin.
