Atypical fibroxanthoma (AFX) is a rare cutaneous spindle-cell neoplasm that occurs most commonly in sun-damaged skin of the head and neck in elderly patients. The lesion usually presents as a solitary nodule, often ulcerated, and usually less than 3 cm in diameter. Wide surgical excision with 1 cm margins used to be the chosen treatment; however, Mohs micrographic surgery is now the preferred method. The authors report a case of AFX in an old man successfully treated with surgical excision.
