We have studied a case of atypical autochthonous cutaneous leishmaniasis which created a series of diagnostic and therapeutic problems. The atypicial nature of the clinical presentation of the lesion and the aspecificity of the histological phlogosis described at the beginning made the differential diagnostic problem more complex. The female patient also proved intolerant to systemic pharmacological treatment with glucantim. Clinical cure was obtained by modifying the therapy even though, owing to the high cast of Ambisome treatment, the patient had to be hospitalised for the entire duration of the treatment.
