The cicatricial pemphigoid is an autoimmune dermatosis principally involving mucous membranes. It develops scars and synechias sometimes causing very important functional disturbances. The bulla is junctional with cleft in lamina lucida; its development is caused by autoantibodies against various not completely characterized antigens. The case of a 70 year-old woman presenting multiple erosions in the oral and ano-genital mucosa with steno-cicatricial evolution is described. The DIF showed a pemphigoid compatible linear fluorescence, while the IFI showed the presence of perikeratinocytary Ab, typical of pemphigus. With the use of human serum as a source of complement and anti-C3 Ab revealer system, it is however demonstrated that this type of perikeratinocytary antibodies are able to fix the complement in vitro, unlike true pemphigus autoantibodies, as confirmed by other similar observations.
