Autochthonous leprosy diagnosed only after surgical intervention for nasal obstruction

The case of a female coming from southern Italy, submitted to surgery for an infiltrative lesion on her nasal septum with the unespected detection of numerous acid-fast bacilli in the excised material, is reported. The subsequent dermatological examination showed many erythematous macules and nodules on trunk and limbs allowing to diagnose a subpolar lepromatous leprosy. Only after the diagnosis was made, the patient revealed that her father and one brother had suffered from lepromatous leprosy.
