Background. Bullous pemphigoid (BP) is an autoimmune disease characterizied by the presence of high levels of IgE. The aim of this study is the evaluation of IgE levels in BP and their role in the pathogenesis of BP.
Methods. Serum total IgE (PRIST) and specific IgE directed against cutaneous structures in 19 BP patients have been studied. Specific IgE were predetermined by indirect immunofluorescence (IFI) and by immunoblotting IB.
Results. The IB, performed on a keratinocytes extract and on the recombinant protein of the major 230 kDa antigen (rBP55), showed the IgG reactivity against BP antigens in 15 cases. Eight patients had circulating antibodies directed only against the 230 kDa antigen; in 6 cases the circulating antibodies recognized the 230 and 180 kDa antigens; only in one case was the reactivity directed to the 180 kDa one. The 230 kDa antigen was recognized by circulating IgE antibodies in 2 out of 19 patients. Immunoblotting against the rBP55 showed a 55 kDa reactivity in 11 of 19 cases with IgG and in 6 cases with IgE.
Conclusions. These data confirm the high levels of total IgE and of anti basal-membrane IgE, the latter directed only against the 230 kDa. The higher sensitivity of IB against rBP55 could depend on the greater exposition of the IgE immunodominant epitopes on the rBP55, which is linear in structure and, because of this, more accessible for the antibodies.
