We report on a 60-year-old female patient with a hepatic cystadenocarcinoma who presented with mild abdominal discomfort and a palpable mass in the right upper abdomen. The diagnostic workup revealed a 7.5×7.5 cm multicystic liver tumor. The patient underwent left hepatectomy and a lymph node dissection. The final histological diagnosis was a primary, moderately differentiated hepatic cystadenocarcinoma (HBCA). The patient remains without evidence of recurrence 18 months after surgery. We report the first case of this rare disease treated at our centre; the clinical presentation, diagnosis, therapeutic options, and classification of hepatobiliary cystadenocarcinomas are discussed.
