Decision making for recurrent mitral paravalvular leak

Repair and closure of a paravalvular leak after multiple redo mitral valve surgery can be extremely difficult and frustrating because, too often, the periprosthetic area is either severely calcified or lacking tissue strong enough to enable the secure placement of direct sutures. We report a rare case of recurrent paravalvular leak, due to a friable and fragile posterior annular tissue at the site of prosthesis attachment, in a patient who had undergone previous close mitral valve procedures.
