Accessory mitral valve (AMV) is an uncommon congenital cardiac malformation usually complicated with the left ventricular outflow tract obstruction (LVOTO). It is extremely rare in adults, so we report a case of AMV in a 33 years old man, highly symptomatic due to LVOTO. Urgent surgical excision of AMV tissue had a favorable outcome and patient stay well and without recurrence six month after surgery.
