We report a rare case of pulmonary combined large cell neuroendocrine carcinoma (LCNEC) in a 74-year-old man. A computed tomographic (CT) scanning of the chest showed a tumor shadow in his right lower lobe which had increased in size over two months from 2 cm to 3.5 cm. CT- guided lung biopsy revealed a diagnosis of non-small cell lung carcinoma. Right lower lobectomy and lymph node dissection were performed. A postoperative histological diagnosis was combined LCNEC with a component of squamous cell carcinoma.
