A 75-year-old woman was admitted to our hospital for repetitive presyncope due to complete atrioventricular block. After an emergent implantation of a temporary pacemaker, a left ventricular tumor was found in transthoracic echocardiography. The patient underwent a successful resection of the tumor through a small ventriculotomy and the pathological diagnosis revealed a papillary fibroelastoma. Cardiac tumors deep in the left ventricle are extremely rare and can often present surgical difficulty. This is the second report in literature of a solitary papillary fibroelastoma arising in the left ventricular septum not in the outflow tract.
