Benign tumors of the papilla of Vater are rare and they can become malignant, but endoscopic biopsy and frozen section miss the diagnosis in high percentage of patients. Local resection is often followed by recurrence. The early and correct diagnosis and the choice of the appropriate treatment, between endoscopic papillectomy, local excision and pancreatoduodenal resection, is very difficult. We describe a patient with ampullary villous adenoma who underwent ampullectomy. Our surgical choice was due to low grade dysplasia demonstrated by endoscopic biopsy and frozen section.
