We report a rare case of transposition of the great arteries with intact ventricular septum which showed a poor response to an anatomically satisfactory balloon atrial septostomy and prostaglandin E1 infusion. Our study reveals that pulmonary hypertension due to a persistent fetal circulation was present. In this patient, urgent anatomic surgical repair has brought about a satisfactory result.
