We report a successful case of urgent mitral valve replacement for acute severe regurgitation due to mitral valve prolapse in a 6-month-old infant. A resected specimen shows that the prolapse was caused by rupture of the infected chordae tendineae of the floppy mitral valve. The progression of mitral regurgitation in patients with floppy mitral valve is gradual. However, rare cases are seen in children.
