We herein report a case of bronchopulmonary sequestration (BPS) with nonimmune hydrops fetalis (NIHF) and mediastinal shift which was diagnosed at 23 weeks' gestation by ultrasonography. The fetus was treated with thoraco-amniotic and abdomino-amniotic shunts at 26 weeks' gestation. One weak later, the decision to deliver the fetus by an emergency Caesarean section was made with based on a diagnosis of maternal mirror syndrome. The baby died of severe respiratory insufficiency at 3 days of life. Only 7 cases demonstrating BPS associated with NIHF have previously been reported in Japan.
