We have reassessed our case histories of one year surgical activity, referring to 170 videolaparoscopical cholecystectomies in 2004. We have particularly examined 51 cases of early discharge during the first day (DANLC). We have compared our results with those of medical literature referable to outpatient videolaparoscopical cholecystectomies. We have described the selection standards of patients standing for DANLC register, on the basis of anesthesiological information and the immediate follow-up. We have been able to ascertain that the mode of discharge during the first day enables us to achieve an expense saving which is about 40% of the services ( according to the national DRG system); besides, it assures the sick a very good margin of clinical safety since in our case histories it has never occurred that events of readmission to hospital came necessarily true, as reported in medical literature especially for videolaparoscopical cholecystectomies in size of 0.1-26%.
