Some cases, fortunately rare, have been described of complications arising due to migration of the mesh plug used for inguinal hernia repair. We report the case of a patient who presented, 4 months after a mesh plug left inguinal hernia repair, with a clinical and instrumental (endoscopy, computed tomography [CT]) picture of stenosing tumor of the sigmoid colon, not confirmed by endoscopic biopsy. After left hemicolectomy, histology revealed the presence of fragments of the mesh, with signs of reaction to a foreign body and fibrosis involving the pericolonic tissue and the sigmoid wall up to the submucosa. It is important to bear in mind the risk of migration of the mesh after hernia repair and we emphasize that particular care must be taken when fixing the mesh to the surrounding fascia.
