Adult ductal system cysts are rare malformative diseases whose origins are still unclear. They are far more common among Oriental people and can be asymptomatic or, more often, develop symptoms due to ductal system lithiasis, pancreatitis, intrahepatic abscesses, portal hypertension. We describe the case of a young woman affected by cystic ductal system ectasia (type IV A, according to Todani's classification), who presented with abdominal pain due to acute pancreatitis; she was treated with endoscopic retrograde cholangiopancreatography and surgical procedure.
