The finding of an appendicular coprolith during a simple X-ray examination of the abdomen is a rare event. Four cases of radiopaque appendicular coprolith associated with acute appendicitis are described. In all these cases, the radiographic finding was confirmed by surgery. This radiological feature is very interesting in cases with difficult differential diagnosis. Abdominal ultrasound examination is also helpful in determining the site of the calculus.
