The subtalar dislocation is a rare injury of the ankle. We describe a case of open unstable medial subtalar dislocation, with a fracture of the fifth metatarsal. The patient is subjected to open reduction of the dislocation under sedation without delay and immobilization for four weeks. After four months, he wanders freely and six months after the shooting sports. We can conclude that the choice of treatment has determined healing, despite the intensity and severity of the injury.
