First Metatarso-Phalangeal Joint Prothesis Toefit-Plus®: Intermediate Results

Toefit-Plus® is a modular implant system for hemi- or total-replacement of the first metatarso-phalangeal joint. Both cementless components of the prostheses, metatarsal and phalangeal, are fixed to the bone by means of a self tapping threaded taper, which ensures an excellent primary stability. The grit-blasted osteophilic titanium alloy surfaces provide ideal conditions for osteointegration, which will ensures the secondary stability of the implant. The modular design of the implant permits its use in a wide range of anatomical and biomechanical first metatarsophalangeal pathologic patters. Choice of the specific type of joint replacement can be made intraoperatively on the basis also of the respect of the gleno sesamoideal complex. Modularity makes it possible to assemble either a hemiprosthesis, when is possible to conserve the metatarsal head, or a total joint replacement. The two types of replacement have different inserts: in the hemiprosthesis the insert is made of CoCr alloy, while total joint replacement has an insert made of PE. From October 2001 to November 2006, 50 Toefit-Plus® resurfacing prostheses were implanted in 45 patients, five patients having bilateral prostheses. In our work we analyze the indications, contraindications and result at mean follow-up of 71 months (max. 108; min: 48) in 35 prostheses.
