The treatment of choice for acute displaced intra-articular calcaneal fractures remains controversial. Many studies have assessed the outcome of treatment of calcaneal fractures, but there is a general disagreement on their management. The surgical treatment using open reduction and stable internal osteosynthesis was made possible by the introduction of new imaging methods such as CT which allowed better detection of the fracture pathology and provided the basis for new surgical strategies. The Authors present a historical review of their treatment options considering 35 years of experience, coupled with the biomechanical rationale for open reduction and internal fixation using an anatomically shaped interlocking calcaneal plate.
