Arthroplasty of the proximal metatarsus-phalanx in hallux rigidus by autologous grafting of chondrocytes in culture: technical notes and preliminary results

Aim. The author presents a personal surgical technique for the treatment of the allux rigidus at the first and second stadium of Régnauld classification. The process foresees the utilization of chondrocytes autograft to recover the epiphysis of the first metatarsus previously remodelled: support to the chondrocytes is a benzyl ester of hyaluronic acid.
Methods. The author illustrates the technique he set up at the beginning of the 2001. In the current year 6 patients have been operated, all females, aged between 37 - 62; 5 classifiable at the II Régnauld stage, 1 at the I one. The follow-up lasts 12 months.
Results. A clinical and radiological check up has been done: the recovery of the dorsoflexion (20 - 50°) is changeable; in any case the deambulation results free and indolent even on tiptoes; much to the patients’ satisfaction.
Conclusion. The technique is quite simple, with a quick learning curve. The price of the graft is high. The first results are encouraging even if the follow up is still short; besides it will be necessary to state which researches to use for an objective check up of the reconstitution of the cartilage tissue (biopsy, NMR with gadolinium).
