The present publication describes V-shaped distal head distal osteotomy according to Jacoby (1973) of the lateral metatarsals. The main indications for osteotomy proper and for resection, meaning by this term the removal from the metatarsals of a bone ''chevron' from between 2 more or less parallel osteotomic lines depending on the case. Two operations are described together with the relevant actions, the type of synthesis and the postoperative course, with possible problems and the real complications observed in the operating series. The reason why, according to the authors, this operation should be considered one of the most valid for achieving the objectives is discussed. In fact, V-shaped distal head osteotomy is: distal metatarsal; extra-articular; an osteotomy which removes the overload by dorsalisation of the metatarsal head or by shortening of the metatarsus; an osteotomy which can be carried out with the same technical procedures in osteotomy and in resection; an osteotomy which permits visual control throughout the operation; an osteotomy which can be ''minimally' synthesised in a stable way.
