We describe the case of a rapid and aggressive malignant hemangioendothelioma of the left talus in a 70-year-old woman with a 20 year history of chronic lymphoid leukemia. The diagnosis was made by radiographs, histology and immunohistochemial studies. The tumor was localized on the talus, which is unusual, and it showed a very aggressive biological behavior which in a short period of time caused a massive osteolysis of the ankle joint and the calcaneous with asymptomatic metastasis on the skull, thorax and left/right distal femora. Only symptomatic treatment was carried out as the patient refused amputation. She died 45 days later after admission to hospital for tumoral investigation. There are few papers which confirm the rare occurrence of these tumors on bones of the foot, and there are none on the talus. Chronic lymphoid leukemia before the appearance of this type of tumor has not been reported.
