The authors propose the use of a particular surgical technique in the treatment of a young patient suffering from Pneumosinus dilatans, a rare pathology characterised by an abnormal increase in the volume of one or more sinusal cavities of the bones of the facial mass. In this patient, the pathology had caused a significant increase in the fronto-glabellar prominence and a very pronounced fronto-nasal angle. A coronal approach to the frontal region followed by osteotomy of the anterior frontal wall together with “star” ostectomy restricted to the extra-cranial compartment was employed to restore the eumorphism of the supra-orbital setting and to reduce significantly the excessive prominence of the frontal bone, with satisfactory final aesthetic results.
