The submental island flap is one of the most versatile flaps available to today’s reconstructive maxillo-facial surgeon. The literature concerning initial surgical experiences with submental flaps is briefly reviewed; surgical anatomy and the technique for raising the island flap are described. Two cases from the authors’ experience are reported: reconstruction of the cheek-nose region and reconstruction of the intra-oral region, both executed using a myocutaneous submental island flap. The chief advantages of this technique are described, emphasizing the aesthetic repair of the donor site (the anterior cervical aesthetic unit) and the high quality of the reconstruction due to the similarity between face and neck in terms of skin colour.
